High Fidelity Wraparound Facilitator I (Licensed)

Position Summary

Contra Costa Youth Service Bureau (CCYSB) is seeking compassionate, clinically skilled, and team-oriented Licensed Clinicians to serve as High Fidelity Wraparound (HFW) Facilitator I staff within our community-based behavioral health programs. This position provides direct services to youth and families involved in intensive care coordination through the High-Fidelity Wraparound model.

The Licensed Facilitator I carries a caseload of youth and families, conducts behavioral health assessments, participates as the clinical member of Child and Family Teams (CFTs), supports treatment planning and crisis response, and collaborates closely with caregivers, County partners, schools, and community providers. This role also participates in Utilization Review (UR) processes and maintains responsibility for completing and signing clinical documentation in accordance with Medi-Cal, EPSDT, County, and agency standards.

This position is ideal for clinicians passionate about providing strength-based, trauma-informed, culturally responsive, and community-centered services to youth and families with complex behavioral health needs.

Essential Duties & Responsibilities

Direct Service & Care Coordination

  • Provide community-based behavioral health services to youth and families enrolled in the High Fidelity Wraparound program
  • Maintain an assigned caseload and coordinate services across systems of care
  • Develop collaborative relationships with youth, caregivers, natural supports, and service providers
  • Facilitate engagement and support family-driven, team-based planning
  • Participate in Child and Family Team (CFT) meetings and multidisciplinary collaboration

Clinical Assessment, Treatment Planning & Documentation

  • Conduct behavioral health assessments and contribute to treatment planning
  • Complete progress notes, assessments, treatment plans, safety plans, and other required clinical documentation
  • Ensure documentation is completed accurately and submitted within required timelines
  • Maintain compliance with Medi-Cal, EPSDT, County, and agency documentation standards
  • Sign and maintain responsibility for own clinical documentation

Crisis Intervention, Safety Planning & Clinical Support

  • Provide crisis intervention, de-escalation, and support to youth and families as needed
  • Participate in safety planning and risk assessment processes
  • Support stabilization efforts and coordination of higher levels of care when appropriate
  • Collaborate with supervisors and leadership regarding high-risk situations and clinical concerns

Utilization Review & Program Compliance

  • Participate in Utilization Review (UR) activities and quality assurance processes
  • Maintain compliance with agency policies, ethical standards, confidentiality requirements, and mandated reporting laws
  • Participate in trainings, team meetings, supervision, and ongoing professional development

Community Collaboration & System Coordination

  • Collaborate with County Behavioral Health, Child Welfare, schools, probation, and community agencies
  • Support integrated care coordination and linkage to community resources
  • Advocate for youth and families across systems while promoting family voice and choice

Minimum Qualifications

  • Master’s degree in Social Work, Marriage and Family Therapy, Counseling, or related behavioral health field
  • Active and unrestricted California LMFT, LCSW, or LPCC license
  • Experience working with youth and families in community-based behavioral health settings
  • Knowledge of trauma-informed, culturally responsive, and strength-based practices
  • Ability to work independently and collaboratively within multidisciplinary teams
  • Valid California driver’s license, reliable transportation, and automobile insurance
  • Ability to provide field-based services throughout Contra Costa County

Preferred Qualifications

  • Experience in High Fidelity Wraparound or intensive care coordination models
  • Knowledge of Medi-Cal Specialty Mental Health Services, EPSDT, and County Behavioral Health requirements
  • Experience facilitating Child and Family Team meetings

Clinical Program Supervisor – Onsite Services

Position Summary

Contra Costa Youth Service Bureau (CCYSB) is seeking a dedicated and experienced Clinical Program Supervisor to provide onsite clinical supervision, staff support, and quality oversight across assigned behavioral health programs. This leadership role ensures clinical integrity, regulatory compliance, and high-quality service delivery aligned with agency standards, County requirements, and Medi-Cal regulations.

The Clinical Program Supervisor provides in-person supervision, coaching, field support, clinical guidance, crisis intervention, and oversight to staff working with youth and families in community-based settings.

Essential Duties & Responsibilities

Clinical Supervision & Staff Oversight

  • Provide BBS-compliant individual and group clinical supervision
  • Offer in-person coaching, observation, and guidance to clinical staff
  • Assign cases, monitor caseloads, and support staff with clinical and operational needs
  • Address performance concerns, documentation issues, and professional development needs
  • Ensure adherence to ethical standards, legal requirements, and mandated reporting laws

Clinical Quality, Documentation & Medi-Cal Compliance

  • Review clinical documentation for accuracy, completeness, and timely submission
  • Ensure assessments, treatment plans, and progress notes meet Medi-Cal, EPSDT, and County standards
  • Conduct routine documentation reviews and support corrective action processes
  • Provide staff training on documentation best practices and clinical expectations

Crisis Response, Risk Management & Safety Oversight

  • Provide onsite crisis intervention and de-escalation support

  • Support high-risk cases and ensure appropriate follow-up and documentation
  • Participate in Serious Event Reporting (SER) processes as required

Program Operations & Field Support

  • Support daily program operations and field-based service delivery
  • Participate in onboarding, training, and orientation of new staff
  • Provide guidance regarding workflow, service expectations, and clinical requirements
  • Collaborate with leadership to support operational efficiency and service quality

Child & Family Team Participation & Community Collaboration

  • Attend and contribute to Child and Family Team (CFT) meetings
  • Collaborate with County partners, schools, community agencies, and multidisciplinary teams
  • Support integrated, strength-based care coordination across systems

Minimum Qualifications

As a Part-Time Bilingual Clinical Therapist for our Wraparound program, you will deliver culturally responsive services and lead a collaborative planning process for youth and families who are referred to through Child Welfare, Juvenile Probation, or County Behavioral Health departments. 

Services will be provided exclusively in the community: in families’ homes, foster homes, schools, other community locations, etc. for youth who could require being placed in out of home care or are stepping down from a higher level of care. 

The primary role is to ensure a sustainable network of unpaid supports (i.e., extended family, mentors, teachers, coaches) are working together toward stability for the youth and family.

The Program

Contra Costa Youth Service Bureau offers “Wraparound” services to children and youth susceptible to placement, and to their families. The Wraparound programs are strengths-based, family-driven, flexible, and creative, with the goal of helping the family develop the skills and supports to prevent or reduce the possibility of out-of-home placement of their child. A family-based team addresses the needs of the entire family, not just the child of focus. This family-based team consists of family members, Contra Costa Youth Service Bureau’s Therapists, Support Counselors, Family Partners, Facilitators, and other community members who have an important role in the family.

Job Overview

The Early and Periodic Screening Diagnosis and Treatment (EPSDT) Wraparound Care Coordinator will work under the Wraparound Practice Model of Care, a framework grounded in individualized, strength-based, trauma-informed, community-based, and culturally responsive values. This role provides intensive and personalized care coordination for children and adolescents referred by Child Welfare, Juvenile Probation, Schools and other Community Based Agencies.

Key Responsibilities

Service Delivery and Coordination

  • Provide intensive, individualized care coordination for children, adolescents, and their families following the Wraparound Practice Model.
  • Responsible for developing, implementing, and maintaining individualized treatment plans that address the unique needs, strengths, and goals of each child and family. This includes empowering families to advocate for themselves while ensuring they receive the necessary services and support to achieve their objectives. The process emphasizes the family’s voice and choice, respecting their culture, values, and preferences. Continuously track service implementation and assess progress, making adjustments to the plan as needed to respond to evolving circumstances or needs. Deliver trauma-informed and culturally intelligent services to youth referred through Child Welfare, Juvenile Probation, Schools and other Community Based Agencies.
  • Facilitate collaborative planning processes and Child and Family Team (CFT) meetings to develop and implement tailored care plans based on strengths, needs, and goals.
  • Ensure a sustainable network of unpaid supports (e.g., extended family, mentors, teachers, coaches) are actively working together toward the stability and well-being of youth and their families.

Community-Based Services

  • Provide services in community-based settings, including families’ homes, foster homes, schools, and other community locations.
  • Actively engage families and youth in the Wraparound process to ensure their voices and preferences guide service planning and implementation.
  • Support families in accessing community resources and navigating local systems to achieve stability and independence.

Documentation and Compliance

  • Maintain accurate and timely Medi-Cal and EPSDT documentation for all services delivered.
  • Keep thorough case notes and ensure compliance with agency and county policies.
  • Document progress and outcomes for reporting and billing purposes.

Professional Development and Collaboration

  • Collaborate with mental health providers, schools, and other community organizations to provide coordinated care.
  • Attend interagency meetings, trainings, and community events to represent CCYSB and foster partnerships.
  • Participate in regular supervision and training opportunities to enhance skills and stay informed on best practices.

Key Qualifications

  • Education:
    • BA in Social Work, Psychology, Counseling, Criminal Justice, or a related human services field is preferred. Equivalent experience may be considered.
  • Experience:
    • Three (3) years of experience working with families in community-based agencies or systems serving high-risk youth.
    • Background or knowledge in mental health services is highly preferred, including an understanding of trauma-informed care, treatment planning, and mental health challenges affecting youth.
  • Skills:
    • Working knowledge of positive child/youth development and patience in understanding the impact of traumatic life situations.
    • Ability to calmly and professionally de-escalate situations involving youth and/or parents.
    • Proven ability to respond effectively in crisis situations.
    • Exceptional multitasking skills with sensitivity to cultural, ethnic, and disability issues.
    • Proven ability to follow oral and written instructions accurately and consistently.
  • Other Requirements:
    • Reliable transportation, a valid California Driver’s License, and current automobile insurance.
    • Flexibility to travel within Contra Costa County for meetings, home visits, and community events.
    • Candidates must successfully pass a comprehensive background check, which may include verification of criminal history, employment history, education, and professional references, as part of the hiring process.
