Cutting-Edge Youth Programs

Empowering Youth, Strengthening Families, Building Communities

At CCYSB, we are dedicated to advocating for and supporting youth and families in Richmond and Contra Costa County. Our programs are designed to bridge service gaps, ensuring that families have access to the resources, care, and guidance they need to thrive.

Our Approach

Family-Centered: We believe in strengthening families by providing them with the tools and resources to overcome challenges and build resilience.

Community-Led: Since our establishment as a nonprofit in 1985, we have fostered strong inter-agency collaborations, ensuring a coordinated and holistic approach to care.

Wraparound

Comprehensive Support for Children & Families

CCYSB’s Wraparound Services provide person-centered, strengths-based, and holistic support for Medi-Cal eligible youth (ages 5-21) and their families. Also known as EPSDT (Early and Periodic Screening, Diagnostic, and Treatment), Wraparound is a collaborative and structured model of care that ensures children and youth with mental health and/or behavioral needs receive the support they need at home, school, and in the community.

How Wraparound Works

A team of professionals, family members, and community partners collaboratively develop and implement an individualized plan tailored to the youth’s unique needs. This plan follows the four key phases of Wraparound:

  1. Engagement & Team Preparation
  2. Initial Plan Development
  3. Implementation & Ongoing Support
  4. Transition & Graduation

Our trained Care Coordinators work closely with families to identify their strengths, challenges, and goals, ensuring that services are culturally responsive, flexible, and effective.

Services Include:

  • Mental health & behavioral support
  • Skill-building & life coaching
  • Family support & advocacy
  • Access to community resources & social services
  • Crisis prevention & intervention
  • Educational & vocational support

Kinship

Supporting Relative Caregivers

Our Kinship Program assists relative caregivers who have taken custody of minor children due to the absence of their birth parents.

Services Include:

  • Case management & advocacy
  • Support groups & peer mentorship
  • Family activities & respite care
  • Transportation assistance
  • Tutoring & academic support
  • Independent Living Skills Program (ILSP)
  • Teen & youth development programs (Girl Scouts, Teen Girls Group)

Kinship Mentoring Program (Family Enhancement Collaborative)

Operating in West Contra Costa County, this initiative promotes relative caregiving as the first option for displaced children. Many children enter foster care because their extended family lacks the resources to care for them. Our program empowers families by ensuring they receive the financial, emotional, and practical support needed to provide stable homes.

CCYSB partners with Contra Costa County Employment & Human Services, Family Stress Center, and Uplift Family Services to expand resources and advocacy for Kinship families.

Differential Response (DR Path II & Aftercare Services)

Early Intervention & Community-Based Support

Differential Response (DR) is a collaborative program between CCYSB and Contra Costa County Children and Family Services (CFS). This initiative ensures that families who have been reported for potential neglect or abuse receive preventative support rather than entering the formal child welfare system.

Our Approach:

  • Community-Based Intervention: Families remain in their own homes and communities rather than entering child welfare.
  • Comprehensive Case Management: Families receive ongoing needs assessments, advocacy, home visits, and referrals to support services.
  • Trauma-Informed & Culturally Responsive Care: We address the root causes of challenges, including domestic violence, substance abuse, and mental health concerns.

Our Goal:

By engaging parents with resources and support early, we help prevent future CFS involvement, strengthen families, and create safer home environments for children.
